By registering, and in consideration of the opportunity to participate in or spectate at events, activities, or programs taking place at the premises/studio (the “Activities”), presented by Sabar Rek Atlanta Inc., Kouyate Jeli LLC, sponsors, partners, and together with its affiliates (the “Organizers”), I, on behalf of myself, my family, personal representatives, wards, dependents, estate, successors, assigns, heirs and next of kin (collectively, “My Attendees”), do hereby enter into this Agreement and agree as follows:

I acknowledge and understand the existence of risks associated with the Activities, including serious bodily injury, permanent disability, exposure to or contracting a communicable disease or virus (including exposure to a bacteria, virus or other pathogen capable of causing a communicable disease or illness), paralysis and death, which may be caused by:

I knowingly and freely assume all such risks and dangers, both known and unforeseen, and I accept full responsibility for any and all injury, illness or damage to my person or property that may result in connection with my participation. I acknowledge that if I believe conditions are unsafe, I will immediately discontinue participation in any activity taking place on the premises.

I agree to abide by the policies and procedures of the Organizers. I acknowledge that I am responsible for supervising guests and family members (including each of My Attendees) who utilize the premises in connection with the Activities, and I agree to be responsible for their actions.

Attendees do hereby release, waive, discharge, indemnify, covenant not to sue and agree to hold harmless for any and all purposes the Organizers, the Premises, their respective employees, officers, agents, sponsors, volunteers, successors and assigns (the “Releases”) from any and all liability, claims, demands, causes of action, suits, losses, damages, property loss or theft, costs or injury or illness, including death, that I may sustain in connection with the Activities, whether caused by the negligence of the Releasees or otherwise.

I irrevocably grant the Organizers and its designees permission to use my and any of my Attendees likenesses in any media now or hereafter existing in connection with my participation in the Activities, for any legal purpose whatsoever, including commercial purposes of the Organizers and its designees and their respective sponsors, licensees, advertisers or broadcasters.

No promise, inducement or agreement not expressed herein has been made to me. I expressively agree that this Agreement is intended to be as broad and inclusive as permitted by the laws of the State of Georgia and that if any portion hereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force ad effect. This Agreement shall be governed by, construed and enforced in accordance with the laws of the State of Georgia without reference to its choice of law rules.

By accepting, I acknowledge that I have read and understand all of the terms of the covenants and agreements, and I am voluntarily waving substantial legal rights, including the right to sue the Releasees.

**Note: If participant is under 18 years old, parent or legal guardian must agree below** For Parents/Legal Guardians of Participants Under 18 Years of Age I hereby warrant, affirm and represent that I am the parent/legal guardian with legal responsibility and the right to make decisions for the below-named participant who is under 18 years of age, and that, in such capacity, I have read this Agreement completely and carefully, understand the risks involved with the Activities, and do hereby consent and agree to his/her participation, release, waiver and acceptance of risk as provided above. On behalf of myself, the above-named participant and my family, personal representatives, estate, successors, assigns, heirs, executors and next of kin, I hereby knowingly, freely and expressly agree to all the terms and conditions of this Agreement and agree to indemnify, defend and release the Releasees of any and all claims by such minor or his/her representatives.
